Cresca · Primary care decision support

Decision support that knows when to stay quiet.

Cresca is clinical decision support for primary care. It watches each patient's own trajectory, speaks only when the evidence supports it, and offers ranked options with their tradeoffs instead of one more instruction.

The problem

Alert fatigue is a design failure.

Most clinical alerts fire on a single value crossing a fixed line. They fire often, they are usually not actionable, and clinicians learn to dismiss them. The one alert that matters arrives looking exactly like the hundred that did not.

Cresca starts from the opposite assumption: an interruption has to be earned. Silence is the default, and every message has to carry enough evidence to be worth a clinician's attention.

How it works

Signal in. Silence, or ranked options, out.

  1. 01

    Watch the trajectory

Follow each patient's measurements and care events over time, against their own expected track.

  2. 02

    Stay silent

With too few points or a weak signal, produce nothing. Most of the time, nothing is the right output.

  3. 03

    Surface once

When a curve bends away and stays away, raise it once, with the evidence that triggered it.

  4. 04

    Rank the options

Lay out reasonable next steps and their tradeoffs. The clinician chooses.

Capabilities

Designed around the clinician's attention.

01

Each patient against their own baseline

Cresca reads change over time, such as a growth curve bending away from its track or screenings that keep slipping, rather than a single reading against a population cutoff.

02

Silent when evidence is thin

When there is not enough signal to support a recommendation, Cresca does not produce one. Abstaining is a designed outcome, not an error state.

03

Ranked options, with tradeoffs

When Cresca does speak, it lays out a short list of reasonable next steps and what each one costs or gains, and leaves the decision with the clinician.

04

Evidence for individual therapy decisions

An evidence layer helps weigh therapy choices for a specific patient, showing the evidence behind each option rather than a single score.

Privacy

No patient data goes to a language model by default.

Cresca is designed privacy first. Personal health information is not sent to a large language model unless a deployment is explicitly configured to allow it. The signal work happens on structured data under the clinic's control.

Status

In development. Pilot planning in Ontario.

Cresca is not available for clinical use. We are planning pilots with primary care practices in Ontario and want to hear from clinicians and clinic leaders who live with alert fatigue every day.

Cresca is in development and is not available for clinical use. It is designed to support clinical judgment, never to replace it.
